4/14/2024 EnchantaVaughn Approved Behavior: At the start of the session, Ashiyah was viewed as displeased, unreceptive, and short-tempered, as it was reported by the mother that Ashiyah was not having a good day. Ashiyah displayed angry outbursts and lashed out at QMHPA when inquired about why she was having a bad day at which mother reports these behaviors last for 10 minutes in duration. Behavior: Ashiyah avoids self-reflection exercise when QMHPA inquired her to deeply think about her reaction to certain situations and what she should do to avoid them. Ashiyah was confronted by QMHPA and asked why she made such a big fuss when she was in the wrong to begin with and when these behaviors are being presented, they last about 10-20 minutes in duration. Intervention: QMHP-C monitored Ashiyah individualized behavior modifications in collaboration with her family. QMHP-C motivated Ashiyah to respond to exercise and express her feelings. QMHP-C offered active listening skills. QMHP-C provided Ashiyah with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP-C encouraged Ashiyah to communicate to her mom when she needs help. QMHP-C praised Ashiyah for staying on task. QMHP-C complimented Ashiyah for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP-C supported Ashiyah displaying positive social interaction with peers and authority. QMHP-C educated Ashiyah about what she can do to make her daily activities more fun and interesting, such as reading her favorite book, drawing in a journal, stretching and closing her eyes and practice the mindful behaviors learned in previous sessions. QMHP-C modeled proper positive behavioral expectations for Ashiyah to encourage additional layers of support and understanding of rules and expectations, as well as, positive pro-social behaviors for Ashiyah to strengthen her understanding of what to do in session, in the home, and in the community. Intervention: QMHP-C encouraged Ashiyah to keep up the good work and continue to listen to her mom, as well as mom’s boyfriend. QMHP-C utilized behavior modification system to aid Ashiyah with following directions given by authority figures. QMHP-C explored Ashiyah’s capability to cope with her stressors and to get back on track. QMHP-C collaborated with mom regarding daily behaviors and performance in the home. QMHP-C reviewed and gave feedback regarding adjustment disorders and its causes and affect toward Ashiyah’s well-being and emotional functioning. QMHP-C monitored, observed, and redirected Ashiyah behaviors as needed. Ashiyah displays normal behavior in therapeutic setting. Ashiyah exhibited a positive attitude toward mom and counselor. Ashiyah verbalizes positive feelings about herself but recognizes areas she needs to work on. Ashiyah smiled and continued to give QMHP-C her undivided attention. Ashiyah was observed in her seat and raising her hand when she wanted to ask a question or contribute to discussion. Ashiyah interacted well in the session and demonstrates good team building skills. Ashiyah displays good attitude by saying “thank you”. Response: Ashiyah did an excellent job by showing interest in the session by asking appropriate questions. Ashiyah admits when she is wrong and apologizes. Ashiyah is more confident in the session, as evidenced by her assertive and proud tone when discussing parts of her day. Ashiyah have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping her hands to herself and staying in designated area. Ashiyah reduces sadness and finds more excitement in learning about her mental illness. Ashiyah is learning to calm down, think first, and respond without anger and frustration. Ashiyah asked appropriate questions concerning the social skills activities. Response: Ashiyah implemented appropriate social interaction skills. Ashiyah displayed good study and organizational skills. Ashiyah was observed complying to authority requests and demands the first time they are given. Ashiyah expresses good statements about her day saying she did her chores, used good manners, and was fair and polite to everyone in the house. Ashiyah continues to make progress with ISP objectives.
